Before the 1960s, abstinence-based detox was the only option for opioid addicts in the United States and produced high relapse rates. However, in 1965, clinicians at Rockefeller University published a report in the Journal of the American Medical Association of the successful treatment of 22 men addicted to heroin with the use of orally administered methadone. Currently methadone must be provided through opioid treatment programs (OTP) in the United States and can not be prescribed in office settings. Although not required by regulation at this time in the United States, people are usually encouraged to attempt other types of treatment methods before entering methadone treatment programs. Since its use began in the 1960s, methadone is still the preferred choice of treatment at the clinics, and is often part of other protocols. The National Institute on Drug Abuse (NIDA) provides the protocol of how to treat addiction with recommended options including medication assisted treatment, cognitive behavioral therapy (CBT), and medical withdrawal management. Other than methadone, newer medications with fewer side effects including buprenorphine and naltrexone have been introduced, relieve drug cravings, block opioid effects, and avoid physical dependence. CBT is an individualized treatment plan that allow therapists to explore patterns of maladaptive substance use to help generate alternative behavior skills.
R-5260 is an opioid drug from the orphine series, which is a derivative of the opioid analgesic norpipanone, but with the piperidine ring fused with a spiro heterocycle. It was discovered by Janssen Pharmaceutica, and is a highly potent opioid around 10x the potency of fentanyl in a rodent model of analgesia undertaken before the MOR and NOP receptors had been differentiated. It is related to other potent opioid derivatives such as dipipanone, bezitramide, R-4066 and spirochlorphine, and is also related to dopamine antagonist neuroleptic drugs such as spiperone and fluspirilene, although R-5260 does not have any dopamine antagonist activity itself.

A pulmonary embolism is a blood clot that becomes lodged in the pulmonary arteries. The majority of emboli arise because of deep vein thrombosis in the legs. The function of the lungs can also be affected by compression from fluid in the pleural cavity pleural effusion, or other substances such as air (pneumothorax), blood (hemothorax), or rarer causes. These may be investigated using a chest X-ray or CT scan, and may require the insertion of a surgical drain until the underlying cause is identified and treated.

On 3 April 2018, the Defence Science and Technology Laboratory announced that it was "completely confident" that the agent used was Novichok, although they still did not know the "precise source" of the agent.

== Use and effects == According to Alexander Shulgin in his book TiHKAL (Tryptamines I Have Known and Loved) and other publications, the dose and duration of 4-HO-DPT are unknown. At a dose of 20 mg orally, there were possibly threshold effects and nothing more. Per Shulgin, there were not enough observations to know what dose would result in activity or what the effects would be. However, the occurrence of threshold effects at a dose of 20 mg was suggestive that "something is nearby". Subsequently, 4-HO-DPT and its presumed prodrug 4-AcO-DPT have been encountered as novel designer drugs, substantiating the notion that more significant effects do indeed occur with 4-HO-DPT at sufficiently high doses. Based on user reports, 4-HO-DPT has an onset of 15 to 45 minutes, a duration of 5 to 8 hours, and produces hallucinogenic effects including psychedelic visuals among others.
